PACE Technical Assistance
Continuum Development Services is a recognized PACE Technical Assistance Center (TAC) and a member of the National PACE Association (Home | NPA | National PACE Association). TACs are available to assist prospective providers in development of successful PACE programs, taking into consideration a broad range of general as well as program-specific factors.
CDS’ President, Dan Gray was responsible for the development of Alexian PACE, now Ascension Living Alexian PACE (Alexian PACE | Program of All-Inclusive Care for the Elderly | Ascension Living) in Chattanooga, TN, one of the most financially successful in the country. CDS has close relationships with several organizations to provide a training center for our clients. Mr. Gray is a former Board member of the National PACE Association and regularly speaks on the development and operation of PACE.
CDS has supported 9 programs, including 4 rural programs in the development and operation of PACE:
- WellQuest River Valley PACE (rural program, estimated opening January 2026), Barling, AK
- Horizon PACE (rural program, 3 sites), Richmond, KY Long-Term Care at Horizon PACE – For Horizon PACE
- Bluestem PACE (rural program), McPherson, KS Bluestem PACE | Bluestem Communities
- LifeCircles (2 sites), Muskegon, MI LifeCircles PACE
- PACE of Southeast Michigan (3 sites) PACE Southeast Michigan: older adult services.
- PACE of Southwest Michigan (2 sites) PACE of Southwest Michigan
- Riverside PACE (5 sites; acquired by InnovAge)
- The Oaks PACE (rural PACE; acquired by Orangeburg Senior Helping Center)
- Carolina Senior Care PACE – EveryAge
CDS has also assisted organizations with Request for Proposal responses, grant applications, acquisition assistance, market analysis, and feasibility studies, as well as operational assessments of existing programs.
